The following are needed for Camp Tahquirz:

Description
This form is required for activities at Camp Tahquitz (Long Beach Council).  This form included a release for the use of fire arms at the camp.

Class 3

 The BSA Personal Health and Medical forms, please read the limitations on each form before selecting a form.  If you have Adobe Acrobat Reader use the links below, if not use the link to the left.
  • Class 1 Medical form is required Scouts and Adults under 40 years of age and must be updated yearly
  • Class 2 is the second part of the Class 2 form and must be completed by a Licensed Healthcare Practitioner.
  • Class 3 Medical form is required for Adults OVER 40 years of age as well as Scouts or Adults engaged in High Adventure or other activities (please refer to the limitation on the form)..
