The following forms are needed for almost all
Scout Activities: |

This form will be filled out once each year.

This
form is almost the same as the form above except it is required
for activities at Camp Tahquitz (Long Beach Council). This form included

The
BSA Annual Health and Medical forms.
All participants should fill out Parts A and C for any event. Part B (the physical examination) should be completed if you are participating in an event that exceeds 72 consecutive hours, such as jamborees, summer camps, and Wood Badge training courses. Part B also is required for participation in a resident camp setting or when the nature of the activity is strenuous and demanding such as service projects, work weekends, or high-adventure treks.

Requests
for reimbursements must be submitted to the Troop treasurer
in writing using the form Request
for Reimbursement or Advance. Receipts must be attached with a
clear explanation of the reason for expense. Reimbursements will generally
be made no later than 7-10 days of receipt. Advances can be made (e.g.,
camping fees) using the same form. |
