Troop 12 Forms
The following forms are needed for almost all Scout Activities:

This form must be completed to join the BSA

This form must be completed to be an Adult leader in Troop12

This information form must be completed by all drivers prior to participating in any Troop or Patrol sponsored event other than days reserved for Troop and Patrol meetings (For example, Campouts, Hikes, Outings like tours of Fun Nights)

This form must be completed prior to participating in any Troop or Patrol sponsored event other than days reserved for Troop and Patrol meetings (For example, Campouts, Hikes, Outings like tours or Fun Nights) This form will be filled out once each year.

To use this form: Click on the Form Link to the left, fill it out online, print it, sign it, and bring it to Scout Master.

This form is almost the same as the form above except it is required for activities at Camp Tahquitz (Long Beach Council).  This form included a release for the use of fire arms at the camp. Click Here for more Tahquitz Forms and Info.

The BSA Personal Health and Medical forms, please read the limitations on each form before selecting a form.  If you have Adobe Acrobat Reader use the links below, if not use the link to the left.
  • Class 1 Medical form is required Scouts and Adults under 40 years of age and must be updated yearly
  • Class 2 is the second part of the Class 2 form and must be completed by a Licensed Healthcare Practitioner.
  • Class 3 Medical form is required for Adults OVER 40 years of age as well as Scouts or Adults engaged in High Adventure or other activities (please refer to the limitation on the form)..

This form must be completed and submitted to the Orange County Council service center 2 weeks in advance of scheduled activity for proper clearance.  It is used for trips of less than 500 miles.  If destination is 500 miles or more one way or outside U.S.A. (local council camp excepted), use National Tour Permit Application, No. 4419.  If backcountry trip, be sure to know BSA Wilderness Use Policy. If the unit contracts with a guide service for a fee, that service must provide a certificate of liability insurance that indemnifies the Orange County Council, BSA. Outings involving climbing must have a qualified guide or CPE cardholder.
Requests for reimbursements must be submitted to the Troop treasurer in writing using the form Request for Reimbursement or Advance.  Receipts must be attached with a clear explanation of the reason for expense.  Reimbursements will generally be made no later than 7-10 days of receipt.  Advances can be made (e.g., camping fees) using the same form.
